J1-J4 Pre-Arrival Instructions This page is only for personnel due to be posted to DMOC. 1. Human Resources (HR) support is provided by the DMOC Admin Office who will be your first point of contact for all HR matters. Please bring this to the attention of your current HR staff in order that your documents can be forwarded to this office in anticipation of your arrival. Furthermore, your medical and dental documents should be forwarded to the SMO or DDA at RAF Northolt as appropriate. 2. Personnel assigned to the JMOT and Photo Section will be required to undertake deployed operations, whilst personnel assigned to the HQ, Trg Sch and Log Sp Wg may be required to undertake deployed operations. 3. You are, therefore, to ensure that you are in date with all your single Service operational training and inoculations, and are to be dentally fit for world-wide deployments. In order to maintain DMOC pre-deployment preparedness states, please bring the following documents with you when you arrive: a. Certificates of single Service annual operational and/or weapon training. b. Certificates of any single Service operational deployment training. c. Respirator Test Certificate (if issued). d. F/Med/965 Operational Medical Record. e. International Certificates of Vaccination or Revaccination against Yellow Fever. f. F/Med/965D Operational Dental Record g. Annual Fitness Test Certificate. h. ID Discs. i. Passport (must be valid for at least 12 months). j. Photocopy of Clothing Record card to show items of deployment clothing and equipment on issue. k. Divisional Docs (RM/RN only). 4. Please fill in this form and return it to DMOC either via post or to the e-mail address at the bottom of the form. |
